NBA To Administer Medical Questionaire To All Participants In Leagues Return

According to a report from Shams Charania from the Athletic, in an effort to place even more of a focus on the health and safety all possible participants in the NBA's "bubble campus," the league will administer a medical questionnaire to gauge the health of all participants as well as determine what their participation level can be.
This will include a survey of the players, coaches, and all traveling parties to Orlando.
Sources: NBA has sent teams a medical history questionnaire for players, coaches, traveling party that physicians will use to determine if members can participate in restart either fully, or restrictions from certain activites, or excused due to pre-existing risk factor(s).
