The Ducks also scored three goals in a span of 37 seconds in Game 4 on Saturday but lost in overtime.
Just over five minutes into Monday's game, Anaheim defenseman Cam Fowler received a pass at the point and skated toward the net before firing a wrist shot past Corey Crawford.
Ryan Kesler scored the second goal 32 seconds later after Chicago turned the puck over in its own zone. Kesler redirected a shot from Jakob Silfverberg that left Crawford flailing.
