Astros Top Prospect Kevin Alvarez Exits Game After Scary Hit by Pitch

The Houston Astros got a scare on Friday evening when the number one prospect in their system was hit by a pitch.

Kevin Alvarez was forced to leave the Single-A Fayetteville game after a frightening sight. The pitch appeared to hit his shoulder to begin with but then ricocheted off his face, in video captured by Astros Future, which keeps up with the Houston farm system.

It appeared that Alvarez was bleeding from his nose following the play and was obviously in pain as he was removed from the game. Fortunately, it did not appear to be extremely serious, but there is no doubt the Astros organization will take the time needed to evaluate their 18-year-old top prospect.

Being ranked No. 1 in the system by MLB Pipeline ahead of Xavier Neyens, Logan Hughes, Albert Fermin and Jack Radel makes this an even more concerning situation.

Depending on which rankings you view, Houston’s farm system is ranked at the bottom of the MLB so a blow like this is something to watch.

Kevin Alvarez Has Advanced in 2026

Alvarez has spent the 2026 season with Fayetteville after he played in the Dominican Summer League in 2025.

The left-handed hitter also plays in the outfield. He entered Friday’s game having participated in 87 games this season where he has a slash line of .266/.348/.439.

He has accumulated 24 doubles, three triples, 10 home runs, 52 RBI and 15 stolen bases.

When you factor his age of just 18 into the picture, he looks pretty impressive.

Houston signed him as an international free agent out of the Dominican Republic in January of 2025. It didn’t take him long to adjust and get the attention of everyone watching.

His development has just continued through the year.

Alvarez was named the Carolina League Player of the Week after he went 12-for-21 with six doubles during a six-game stretch in May. He was also the first Fayetteville player in history to record multiple hits in every game he played in during a series.

To everyone’s delight, his power is starting to show up as well.

He hit his 10th home run of the season on Wednesday when he got a two-run shot. Given that he hit just two runs in 47 games when playing in the Dominican Summer League last season, the 10 he has hit this year is quite a jump.

His overall slugging percentage has actually dropped a little from .455 to .439 as he has faced more advanced pitchers so he hasn’t just exploded on offense. What has changed, though, is how often Alvarez is putting the ball out of the park.

Last season he homered once every 23.5 games. This season, he is doing it once every nine games.

That’s an encouraging development for an 18-year-old prospect.

So, for the number one prospect in the organization to take a blow to the face, the gasps were warranted. For now, Houston will just hope that a very frightening-looking play ultimately just turns out to be scary rather than significant.
